Care briefing

# Make room for questions about harm.

The useful question is about the exact product and your own health. Ask what needs to be checked before care starts and who handles a problem afterward.

FDA is short for Food and Drug Administration, the US agency that reviews medicines.

The product

#### Before care

Ask about the exact product, how its label is explained and which questions a pharmacist can answer.

#### If a problem appears

Keep the product and pharmacy names, label and delivery details together for the clinician or pharmacist.

A compounded preparation

#### Before care

The FDA says poor mixing can leave germs in medicine or the wrong amount of active ingredient, the part that does the intended work.

#### If a problem appears

As of September 2026, the FDA has reports of patients measuring the wrong amount of compounded semaglutide for injections and health professionals calculating amounts incorrectly. Some reported problems required hospital care.

Storage

#### Before care

Peptide products have different storage needs; some require cold storage and others do not.

#### If a problem appears

Ask the pharmacist about a delivery that does not match the storage instructions for that product.

Ask the pharmacist to explain the measuring and storage instructions for your exact medicine until you can repeat them back.

## Ask about your own health before a decision

Bring up other medicines, supplements, allergies and plans for pregnancy or breastfeeding. Ask what information the clinician needs to assess the proposed option.

The FDA has not reviewed a compounded product’s pregnancy or breastfeeding information as it has for an approved medicine, as of September 2026.

Ask what is known for the exact product. An answer about a different medicine leaves your question open.

## When help is urgent

MedlinePlus, a health service from the US National Library of Medicine, says severe allergic symptoms such as trouble breathing require emergency care.

A severe allergic reaction may develop quickly and threaten life, according to MedlinePlus.

## Reporting a medicine problem

Patients can send medicine-problem reports through MedWatch, the FDA’s reporting service.

A report does not by itself establish that the medicine caused the problem.

Keep the prescriber’s contact details as well as the instructions for making a report.

Sources for this briefing

Choose the source for your question. For a doctor’s licence, use the state licence search. For a medicine’s approval, ask to see the FDA approval and approved label for that exact product.
